encontrar de los profesionales del laboratorio,
se centran principalmente en el trato al
paciente, considerando que las políticas
gubernamentales han realizado un cambio,
incluyendo políticas de no discriminación,
trato digno, que implica el respeto a sus usos y
costumbres, de alguna manera continua el
trato preferencial a cierto tipo de pacientes,
ejemplo, por parentesco, amistad, influencia
de jerarquía, condición social y toda situación
de privilegio; otras denuncias van en el sentido
de la no coherencia de los resultados con la
clínica médica, y muchas otras más en la no
coincidencia de reportes entre resultados de
diferentes laboratorios (2,3).
El profesional bioquímico, como cualquier
profesional de salud va en la búsqueda de la
perfección de su diagnóstico, la institución
donde trabaja, la situación económica del país
y la falta de coordinación con el área médica,
enfermería y otros servicios administrativos
no le bridan las condiciones necesarias que
requiere para el desempeño de sus funciones.
Las nuevas leyes y su difusión en medios
sociales hacen que la población lo considere
como un generador de resultados de muestras
biológicas, que no se le permite equivocación,
pero el avance en las investigaciones nos
muestran que existen muchos factores
relacionados al paciente, como los factores
genéticos, determinantes sociales que afectan
de gran manera a la calidad de la muestra,
también no contar con el equipamiento
necesario y reactivos garantizados hace que no
se tengan resultados confiables. Es un
inconveniente al momento de pretender
reportar resultados que sean cien por cien
fidedignos, todo lo anteriormente mencionado
nos lleva a evaluar la necesidad que se tiene
de crear instrumentos para la auditoria.
En un trabajo publicado en 1997, Mario
Plebani y Paolo Carraro sostenían que el foco
del control de calidad se centraba hasta
entonces principalmente en las actividades
analíticas del laboratorio clínico. Sin embargo,
la tarea del laboratorio comienza en el
momento de la recepción de la solicitud
médica y abarca todos los procedimientos que
se desencadenan desde entonces hasta la
entrega del resultado al médico. No debe
descuidarse por lo tanto, buscando
oportunidades de mejora de la calidad, el
seguimiento de las fases preanalítica y
postanalítica. De hecho, diferentes autores han
encontrado que la frecuencia de errores en
estas fases es mayor que en la analítica. Al
respecto, Pierangelo Bonini y colaboradores
revisaron los resultados encontrados en la
bibliografía en un período comprendido entre
enero de 1994 y junio de 2001, reportando un
promedio de 55% del total de errores para la
fase preanalítica. Por lo general todos los
instrumentos que se tienen disponibles van en
el sentido de hacer una vigilancia en el mismo
proceso del desempeño profesional como la
auditoría clínica para vigilar y reducir la
magnitud de errores analíticos en el
Laboratorio y respecto a instrumentos en
casos de negligencia por parte del personal
bioquímico, no se pudo encontrar mucha
información en la bibliografía al respecto, las
que existen se refieren a casos de errores,
manejo de incidentes y no conformidades en el
laboratorio clínico, con protocolos e
instrumentos internos del laboratorio (4,5).
Evaluación y Auditorías en laboratorios
Una evaluación puede definirse como el
análisis sistemático de una parte (o a veces la
totalidad) del sistema de gestión de la calidad
para demostrar a todas las personas
interesadas que el laboratorio cumple con los
requisitos reguladores, de acreditación de los
usuarios externos. Los laboratorios de nivel
central normalmente están familiarizados con
los procesos de evaluación, puesto que la
mayoría se habrá sometido a algún tipo de
análisis por parte de un grupo externo. No
